After installing the plugin. You’ll have a new menu in your admin page ”GAPlugin”.
It will have a ”form” page.

Then you just have to use the shortcode in your page to display it:
– form shortcode is: [GAP-Form]

Install the plugin.
Activate the plugin through the ’Plugins’ menu in WordPress.
In the ”GAP-Plugin” Menu in your admin panel, you can select your questions.
Finally place the shortcode [GAP-Form] in your page.
